How'd he get that shot?

This is from a Wedding in December in Woodinville. The Beautiful Bride is Alena. Sometimes as I look at other photographer's work for inspiration. I go how that heck did they do that.

Camera - almost doesn't matter although having good equipment helps with sharper images
Lens - 16-35 Mark ii, it was tight area and I wanted to incorporate the tall windows
Light - One Canon 580ex Mark ii
Diffuser - One small bounce umbrella and a light stand
2 Assistants - Vince and Bryan
Triggers - Alien Bees Cybersyncs
